Iterative Care Model

- Problems:
- Many existing models are:
- Limited to administrative issues like Admission, Hospital Stay,
Discharge, in one particular hospital.
- Limited to descriptive ands narrative approaches, according to
motivation sometimes exclusively based on data collection for
epidemiological purposes.
- Focusing on only one "clinical path" like for example hip
replacement with diagnose, surgery, recovery.
- Too large, masking the essential aspects.
- Focusing on the needs of specialized care providers.
- Objectives:
- Try to make a more generic model about the care process, from a life
long patient point of view and taking advantage of telecommunication
tools, regardless of organizations boundaries,
- Appoaches:
- Start from the patient needs and try to solve his/her problems.
- Care to be seen as a continuous iterative process.
- Care tean approach: modern care require the participation of several
specialized actors, who need to share information about the
patient.
- Useful information could be derived for epidemiological surveys, but
this is here not the primary goal.
